Willowbrook Health & Home
Willowbrook Health & Home Services is licensed as a “Professional Support Service” agency to provide skilled rehabilitative therapy services anywhere in the State of Tennessee. Under a contract with the State, Willowbrook therapists serve the special needs of individuals with intellectual disabilities and developmental disabilities (ID/DD) throughout middle Tennessee. We assist these individuals in home and community settings identifying therapy approaches that help them to achieve their maximum function without compromising safety and comfort. |

Detailed Available Services
Our therapists focus on finding motivating factors that bring out the untapped potential of these individuals as
their therapy progresses. They concentrate on increasing functional independence as well as community
integration through various activities that are of interest to the individual. The therapists work on small steps
towards each functional goal. Each individual has a one-year plan of care, which is reassessed annually by the
therapist. Overall, our primary goal is to increase the quality of life for the individuals being served and strive to
be the preferred therapy provider in middle Tennessee. |
