How do I qualify for

You must have a medical condition
that meets the criteria for home care. There must be the need for a
skilled practitioner, such as a nurse or physical therapist. A Medicare
patient must be medically homebound (absences from home are infrequent,
of short duration, or to receive medical care).

Who can benefit from
Willowbrook Home Health Care?
Patients recovering from surgery, an
illness or an accident can benefit from home health.
How long will I receive
home health?
After your case is reviewed, you are
eligible for a sixty-day period of care if you are a Medicare patient.
If you still require Willowbrook Home Health Care after that sixty-day
period, your case will be reassessed and you may be allowed a sixty-day
extension. The process of treatment and review will go on as long as is
necessary. If you have commercial insurance, the case manager at your
insurance company will determine how long you will receive
services.
How do I pay for
Willowbrook Home Health Care?
Payment options include: Medicare,
Medicaid, VA, Worker’s Compensation, private insurance, and private
payment. When vital sign telemonitoring is considered, there is no
additional charge to Medicare patients.
What should I do to initiate home health
services with Willowbrook Home Health Care?
You must be
referred to Willowbrook Home Health Care by your physician or his/her
agent (nurse, hospital discharge planner, case manager,
etc.)
How will I know which services I will receive
from Willowbrook Home Health Care?
Your physician
will order specific services at the time of referral. A registered nurse
or a therapist will schedule an initial visit to evaluate and assess
your medical condition and ongoing needs. After the evaluation, the
nurse or therapist will contact your physician to discuss your plan of
care. The nurse and/or therapist will inform you of the services that
will be provided.
How often will a staff member from Willowbrook
Home Health Care visit me?
The frequency
and duration of each discipline will be determined based on the
care plan established with your physician.
How will I know when a staff member from
Willowbrook Home Health Care will visit?
Before the end
of each visit, the staff member will inform you of the date for the next
visit.
Can I get a home health aide for personal
care?
The need for
home health aides depends on your current functional status and your
ability to improve your functional status. You must also have a
qualifying skill provided (i.e. nursing, physical therapy, occupational
therapy or speech therapy).
Will I have to pay for any medical supplies
and/or equipment?
Medical supplies
are typically included as part of the visit and will not be billed to
you. The medical equipment company will bill Medicare or your insurance
company for any equipment ordered by your
physician.
Does Willowbrook Home Health Care have staff
available at night, weekends and holidays for emergencies?
Yes. Willowbrook
Home Health Care has a nurse on call at all
times.
Does Willowbrook Home Health Care perform
background checks on the staff who provide patient care?
Yes. Willowbrook
Home Health Care recognizes the importance of employing honest, credible
and compassionate caregivers. A criminal background check is conducted
on all patient care staff prior to employment. Willowbrook Home Health
Care also conducts background checks on all office and support staff
prior to employment.
